Please fill out the following forms for those going to Lutherock this summer.
All campers should fill out the LCC Youth Info Form and Lutherock Camper Health Form. If the camper needs to take any medications, please also fill out the Lutherock Medications Form.
For campers attending either Middle School Sampler or High School Sampler, please fill out the USA Raft Waiver and High Mountain Expeditions Waiver.
For campers going on the Whitewater Week, please fill out the Appalachian Wildwaters Waiver.
